For Vets: How to refer a client to me
Please complete the referal form below and upload a copy of the dog’s medical history
I will then contact our mutual client with details of cost and an enrolment form for them to complete
A copy of my assessment and plan will be sent to you (the vet) once I have completed the assessment visit.
This may include any observations from the consultation I have advised the client discuss with you.
You are always welcome to contact me to discuss any aspects of current or potential cases.
